BH/SUD Navigator

Seeking a Behavioral Health/Substance Use Navigator in Mahoning County.

Responsibilities include, but are not limited to:

  • Providing crisis support services and follow up
  • Working with clients being discharged from emergency departments in efforts to identify service needs
  • Providing direct and indirect services for adults as part of a multi-disciplinary team
  • Linking/enrolling clients into services.
  • Deliver early intervention and treatment services for persons with substance use disorders and those at risk of developing these disorders.

Must have Bachelor's Degree and CDCA (or be able to obtain CDCA). In addition, must have one year of related experience working with people who are severely mentally disabled and/or dually diagnosed, knowledge of psychiatric disorders, and significant familiarity with community systems and agencies.

Must be willing to complete outreaches in the community. Must have and maintain excellent driving record. Prior experience working with clients in a community based setting preferred.

Must have NPI and Medicaid Provider numbers or willing to obtain such.
