Behavioral Health Consultant - FT - Stark County, Ohio

Coleman Health Services is a nationally recognized not-for-profit provider of behavioral health and rehabilitation programs to foster recovery, build independence, and change destinies for individuals, families, and businesses in our community.  We are seeking a licensed therapist to provide services to children and adolescents at a pediatric primary care practice in Canton, Ohio. Eligible for $5,000 sign-on bonus!

Duties include, but are not limited to

-Providing consultation with medical professionals related to mental health and substance use 

-Developing and fostering partnerships with medical staff at the identified clinic

-Assessing for client needs and emergent concerns 

-Completing standardized screening assessments mutually agreed upon by the medical practice

-Providing client consultation, diagnostic assessment, and short-term counseling 

-Making behavioral health treatment recommendations 

-Facilitating referrals to appropriate services based on identified needs

-Engaging with parents/guardians of persons served when appropriate and required

-Assisting with key social determinants of health needs, including food, income, and housing

-Completing follow-up calls and care to assess progress, barriers, and goals to promote wellness and recovery

Qualified candidates must be licensed as an LSW with Master's or LPC.  Candidates must also have a NPI number, an Ohio Medicaid number, and a CAQH account created. Prefer someone with previous experience in child and adolescent behavioral health or other medical setting and independently licensed counselor or social worker.
