Care Coordinator/Case Manager (OhioRISE)

Full-Time | Paid Training | Supportive Team Environment

We are hiring a Care Coordinator / Case Manager to support children, youth, and families with complex behavioral health and social service needs. In this role, you will help families navigate systems, coordinate services, and stay connected to care. This is a support-intensive, structured role with clear expectations, regular supervision, and active leadership involvement. Prior care coordination experience is not required - training and coaching are provided. Many of our successful Care Coordinators started without prior care coordination experience. We value reliability, organization, sound judgment, and follow-through over "perfect" resumes.

Pay & Perks

  • $44,000-$49,000 annually, based on experience and education

Key Responsibilities

  • Coordinate behavioral health and social services across providers, schools, and community systems
  • Support families with care planning, follow-up, and service connections
  • Maintain timely, accurate documentation in an electronic health record (EHR)
  • Monitor safety, risk, and follow-up needs with supervisory guidance
  • Communicate regularly with families, providers, and partner agencies
  • Track tasks, deadlines, and follow-ups to ensure continuity of care
  • Participate in team meetings, training, and individual supervision

Qualifications & Skills (Training Provided)

You may be a strong fit even without prior care coordination experience if you have:

  • Strong organization and time-management skills
  • Clear, professional written communication
  • Comfort working with families experiencing stress or crisis
  • Ability to follow established processes and documentation standards
  • Willingness to learn, accept coaching, and work as part of a team
  • Reliability, accountability, and good judgment

Experience That Translates Well

We strongly encourage applicants with experience in:

  • Case management, child welfare, foster care, housing, or domestic violence services
  • Behavioral health, mental health technician, or community support roles
  • Social work, psychology, sociology, education, or human services
  • Customer-facing roles requiring documentation, coordination, and follow-through

Education: Bachelor's degree preferred (social work or related field), but relevant experience and demonstrated skills are highly valued.

Location & Travel

This role is community-based within Ashland, Ohio, with mileage reimbursement provided for work-related travel.
