Coleman Health Services, formerly Coleman Professional Services (CPS), is a nationally recognized not-for-profit provider of behavioral health and rehabilitation programs to foster recovery, build independence, and change destinies for individuals, families, and businesses in our community. We are seeking a team player for our Case Management Program in Auglaize and Allen Counties, Ohio.
Responsibilities include establishing client outreach plans, providing support in development and accomplishment of goals, and establishing community/client relationships. Assist clients with daily living skills, medication monitoring, and following their Individual Service Plans. Must have Associates in Human Services or related field; be willing to work a flexible schedule to meet the needs of our consumers; have experience working with severely mentally disabled individuals; and have and maintain excellent driving record. Individual must have the following: a NPI number, and eligibility to apply for an Ohio Medicaid number. BA/BS in Social Services preferred.
