We are seeking a Case Manager to provide care coordination and other case management services to individuals in Portage County who have mental health and substance use disorders.

Duties include: 

• Coordinate ongoing care with internal and external healthcare providers
• Assist with symptom monitoring and coping skill development
• Assist with making healthcare, service, and community resource referrals

Qualifications: 

• BA/BS in mental health/related field or three years' related experience
• Have/obtain a NPI number and an Ohio Medicaid number
• Have/maintain an excellent driving record

Preference given to the following candidates:

• BA/BS in mental health/related field
• Someone with a CDCA
• Experience in case management
• Experience working with adults in a community mental health setting
