Coleman Health Services, a nationally recognized not-for-profit provider of behavioral health and rehabilitation programs, is seeking several Case Managers (CM) to join our team. We have multiple positions in various disciplines including General, Social Services, ACT and CTI case management in Stark County, Ohio. All CMs assists at risk clients with accessing community based resources, housing, employment, making appropriate referrals and assisting with maintaining scheduled appointments.
Qualified candidates are required to have a BA/BS. Preferred to have 1 year experience in case management, mental health or a related field. Must be eligible to obtain an NPI number and an Ohio Medicaid number for billing purposes, which Coleman can assist you in obtaining. Must have and maintain an excellent driving record and vehicle for transporting clients in the community.
