Case Manager - FT - Stark County, Ohio

The Case Manager will assist with skills building and accessing community based resources, housing, and employment by making appropriate referrals and linking the individual with ongoing mental health services. 

Qualified candidates are required to have a BA/BS. Preferred to have 1 year experience in case management, mental health or a related field. Must be eligible to obtain an NPI number and an Ohio Medicaid number for billing purposes, which Coleman can assist you in obtaining. Must have and maintain an excellent driving record and vehicle for transporting clients in the community.
