Coleman Health Services, formerly Coleman Professional Services (CPS), is a nationally recognized not-for-profit provider of behavioral health and rehabilitation programs to foster recovery, build independence, and change destinies for individuals, families, and businesses in our community. We are seeking a case manager in Trumbull County, Ohio, to provide case management services to individuals with severe and persistent mental health issues who may also have a substance abuse disorder.
The Case Manager will assist with accessing community based resources, housing, employment and linking the individual with ongoing mental health and/or substance abuse services. The Case Manager will work with Probation, Parole, Courts, Social Security, DJFS and other providers. Must have a high school diploma; prefer someone with a BA in a related field; one year related experience and knowledge of community resources. Must have and a maintain a clean driving record.
Must have an NPI number and Medicaid Provider Number or be willing to obtain such.
