Coleman Health Services is seeking a full time Case Manager to provide case management services to individuals with severe and persistent mental health issues who may also have a substance abuse disorder in Trumbull County.

Job Duties of Case Manager includes, but not limited, to:

• Assist with accessing community based resources, housing, employment and linking the individual with ongoing mental health and/or substance abuse services.
• Transition clients and families into ongoing care
• Work collaboratively with treatment teams internally and externally to ensure safe transitions

Qualifications of the Case Manager

• Bachelor's degree in a social service-related field or three-years' experience
• Willing to get NPI and Medicaid Billing number (we'll help!)
• Maintain excellent driving record
