Assertive Community Treatment Team (ACT) is an evidence based program that engages with clients who are most at risk. Shared caseload approach and daily team and individual support is offered. The ACT team assists clients with accessing community based resources, housing, employment, making appropriate referrals and assisting with maintaining scheduled appointments.
Must have BA/BS in mental health or related field and be passionate about working with our clients; have a NPI and an Ohio Medicaid number; have and maintain an excellent driving record; and be able to work a flexible schedule including an after-hours on-call rotation.
