The Case Manager will assist with accessing community based resources, housing, employment, making appropriate referrals and linking the individual with ongoing mental health services. Must have BA/BS in mental health or related field or three years experience in mental health or related field; have/obtain a NPI number and an Ohio Medicaid number; and have/maintain an excellent driving record. Experience in case management or experience working with children and young adults in a community mental health setting preferred.
