Case Manager (CTI) - FT - Stark County, OH

Coleman Health Services, a nationally recognized not-for-profit provider of behavioral health services, is seeking an experienced Case Manager to join our Crisis Services team as a Critical Time Interventionist (CTI). Critical Time Intervention is a strategy of engaging persons who are admitted to a psychiatric hospital for the purposes of optimal discharge planning, transition back into the community, and engagement with outpatient treatment services.  CTIs assess individual's needs and link them to appropriate community-based resources including housing, employment, ODJFS, and treatment services for a period of up to 6 months. 

Case Manager (CTI) Qualifications:

·  BA/BS in Human Service field

·  Minimum of one year experience working with the SMD/AoD population

·  Must have or willing to obtain a NPI and Ohio Medicaid number

·  Must have an excellent driving record

·  Must be willing to complete and have favorable results for criminal background check, drug/alcohol screening, and driving check if selected for the position
