Case Manager, Mobile Response

Coleman Health Services is a nationally recognized not-for-profit provider of behavioral health and rehabilitation programs to foster recovery, build independence, and change destinies for individuals, families, and businesses in our community. We are seeking a case manager to assist with rapid engagement and crisis stabilization of adults seen by the Mobile Response Team in Stark County, Ohio. Emphasis is placed on keeping them safe in their nature environment, increased coping skills, accessing/navigating community resources, and linkage to outpatient services.

Duties include:

  • Providing short-term, community-based case management to assigned adults
  • Safety planning and increasing healthy coping skills
  • Developing treatment plans
  • Assisting with accessing supports, resources, and outpatient services that will help stabilize the crisis and keep them safe in the community

Qualified candidates will have their Bachelor's degree in a related field or three years related experience (includes internships and volunteer experience); have and maintain an excellent driving record; and familiar with trauma informed care, community resources, motivational interviewing, crisis intervention, and case management approaches.

Prefer someone with prior case management experience. Must be willing to complete and have favorable results for criminal background check, drug/alcohol screening, and driving check if selected for the position.
