Case Manager (Youth) - FT - Stark County, Ohio

We are seeking a passionate individual to provide short term case management services to youth and their families in their natural environment, schools, and various community settings. 

Duties include: 

?Provide short-term, community-based case management to assigned families following a behavioral health crisis

?Safety planning and increasing healthy coping skills

?Develop treatment plans

?Assist with accessing supports, resources, and outpatient services that will help stabilize the crisis and keep them safe in the community

?Function as part of an interdisciplinary team and develop and maintain effective relationship with community professionals

Qualifications: 

?Bachelor's degree or Master's degree in behavioral health or related field

?Must be eligible for an NPI number and an Ohio Medicaid number

?Acceptable driving record

Preference given to the following candidates: 

?Prior case management experience

?Understanding of child and adolescent development, behavioral health, and community resources 

?Past or current experience in motivational interviewing, crisis intervention, and trauma informed care

This crucial position is available thanks to a nearly $4 million SAMHSA grant that Coleman received to implement Certified Community Behavioral Health Clinics (CCBHCs) in our service areas of greatest need.
