AOT Monitor - FT - Stark County, OH

Coleman Health Services is a nationally recognized not-for-profit provider of behavioral health and rehabilitation programs to foster recovery, build independence, and change destinies for individuals, families, and businesses in our community. We are seeking an AOT Monitor located in Stark County, OH.

Duties and Responsibilities:

  • Serves as the liaison for Stark MHAR and monitors all aspects of the program to ensure it is being carried out effectively.
  • Assists in developing, implementing, and improving the quality of the AOT program.
  • Develops relationships with key community partners, including, but not limited to, Stark County Probate Court, Heartland Behavioral Healthcare (HBH), community hospitals, law enforcement, crisis services, mental health service providers, families, and advocates.
  • Responds to questions or concerns from the public, key community partners, respondents, and treatment team members.
  • Recommends AOT at HBH bed day meetings, as appropriate.
  • Receives and reviews AOT referrals.
  • Meets with referents to review AOT program.
  • Meets with natural support to provide education regarding AOT program.
  • Submits required documentation to Stark County Probate Court for review.
  • Attends Initial AOT hearing as well as status hearings.
  • Keeps and reports pertinent data of admissions, discharges, and referrals.

AOT Monitor Qualifications:

  • Minimum - Bachelor's Degree in Human Service field and 2-5 years of experience working with the SMD population in an outpatient treatment setting.
  • Must be willing to complete and have favorable results for criminal background check, drug/alcohol screening, and driving check if selected for the position.
