Coleman Professional Services, a nationally recognized not-for-profit provider of  behavioral health and rehabilitation programs, is seeking a case manager to assist with rapid engagement and crisis stabilization of youth and families seen by the Mobile Response Team in Trumbull County, Ohio. Emphasis is placed on keeping the youth safe in their nature environment, increase coping skills, accessing/navigating community resources, and linkage to outpatient services.

Duties include:

• Providing short-term, community-based case management to assigned families
• Safety planning and increasing healthy coping skills
• Developing treatment plans
• Assisting with accessing supports, resources, and outpatient services that will help stabilize the crisis and keep them safe in the community

Qualified candidates will have their bachelor's degree in a related field or three years related experience (includes internships and volunteer experience); have and maintain an excellent driving record and be familiar with child development, family systems, trauma informed care, community resources, motivational interviewing, crisis intervention, and case management approaches. Prefer someone with prior case management experience.
