Clinical Coordinator - Embrace Moms Program

Coleman Health Services is a nationally recognized not-for-profit provider of behavioral health and rehabilitation programs to foster recovery, build independence, and change destinies for individuals, families, and businesses in our community. We are seeking a team player for our Embrace MOMS program to coordinate medical and behavioral health services, coordinate material goods inventory, and works with pregnant mothers to provide pregnancy and parenting education and support in Allen County and surrounding areas. Pay $52,000/year.

Duties of Clinical Coordinator Includes:

  • Keep client documentation and fulfill reporting obligations.
  • Deliver education on pregnancy and parenting that aligns with established programs.
  • Offer educational services that are consistent with contracts and stakeholder requirements.
  • Facilitate access to counseling, assessment, case management, or other clinical services for interested and eligible clients in the Embrace MOMS program.
  • Organize meetings and conduct outreach efforts for stakeholder groups.
  • Provide orientation, training, and mentorship to staff and volunteers while managing their schedules.

Qualifications of Clinical Coordinator:

  • Bachelor's in social work or counseling and an LSW/LPC.
  • Have and maintain NPI and Medicaid provider number.
  • Excellent verbal and written communication skills.
  • Able to work effectively and collaboratively as a team, interacting with clients, vendors, and employees. Able to exercise balanced judgement in evaluating situations and making decisions, and to handle difficult or confrontational situations in a calm, consistent, and equitable manner.
