We are seeking an individual to work with homeless people in shelters, crisis stabilization units, jails, hospitals, detoxification programs, rehabilitation programs, health clinics, missions, on the streets and other social service agencies in Trumbull County.
Duties will include:
Must have one year of experience in the mental health field or as a Case Manager; two years' experience with a Bachelor's Degree is preferred. Access to a personal vehicle and willingness to transport clients in your personal vehicle is required along with the ability to drive a company-owned vehicle. Must have familiarity of community resources, work effectively on a team, and interact with internal and external customers. Must be able to work a flexible weekday schedule and potentially work in the evening. Must have and maintain a clean driving record.
Must have, or be willing to obtain, an NPI and Medicaid Provide Number.
