CME Intake Specialist (Youth/Young Adults) - Behavioral Health (OhioRISE)

Be the First Connection for Youth and Families

Play a critical role in connecting youth and families to meaningful behavioral health services through OhioRISE. Coleman Health Services is seeking a skilled and compassionate professional to serve as a CME Intake Specialist, acting as the primary point of contact for referrals and ensuring timely, high-quality assessment and access to care. This position is located in our Akron, Ohio, office and serves Summit County.

What You'll Do

• Serve as the primary point of contact for all OhioRISE youth referrals across assigned counties
• Conduct CANS assessments
• Coordinate intake, enrollment, and triaging of referrals to determine immediate needs
• Support youth hospitalized due to behavioral health conditions by assisting with intake and transition planning
• Communicate proactively with referral sources to gather collateral information and provide status updates
• Ensure a smooth, supportive warm handoff to the assigned Care Coordinator following enrollment
• Maintain timely, accurate documentation and tracking of referral and intake activity

Qualifications

• High school diploma or equivalent
• 3+ years' experience providing community-based services to youth and their families
• Valid driver's license
• Ability to obtain NPI and Ohio Medicaid numbers (assistance available)
