Jail Liaison - FT - Stark County, Ohio

Coleman Health Services is a nationally recognized not-for-profit provider of behavioral health and rehabilitation programs to foster recovery, build independence, and change destinies for individuals, families, and businesses in our community. We are seeking a Jail Liaison, that will be responsible for coordinating and engaging clients in MH and/orSUD services in Canton, OH.

Responsibilities of the Jail Liaison Includes:

·  Screen and evaluate risks of clients and connect to services.

·  Initiate discharge planning for clients to other area providers.

·  Develop relationships with key personnel at local and state jail(s), courts, and probation offices.

·  Coordinate release planning, including transportation, for all persons being released from jail with a behavioral health and community provider(s). 

·  Provide advocacy and community outreach after release from jail within timeframe established via policy, as well as, tracking Performance Target Outcomes and Weekly Provider Reporting plus reporting to provider agencies on all interviewed inmates daily.

·  Communicate with service providers about current legal status, treatment provided while incarcerated and strengths and barriers to release.

Jail Liaison Qualifications:

·  Candidates must have BA/BS in mental health or related field or 1-2 yrs. of relevant experience with case coordination and case management with the Adult Behavioral Health population.

·  Must be eligible for an NPI number and an Ohio Medicaid number - we will help you!

·  Must have and maintain excellent driving record.

·  Must be willing to complete and have favorable drug and alcohol screening if selected for the position.
