We are seeking a full-time licensed practical nurse in our Warren, Ohio location in Trumbull County.  Candidate must be detailed oriented, able to consult with psychiatrists, triage phone calls, monitor medication, have excellent assessment skills, and be able to problem solve to obtain medications.  Must be able to work with a team to help promote health and remove barriers to care for psychiatric and physical health.  Must be an Ohio Licensed Practical Nurse and have the following: an NPI number and an Ohio Medicaid number. Experience with psychiatric nursing, telemedicine, electronic medical records and prescribing practices preferred.
