Manager of OhioRISE Case Management/Care Coordination - FT - Kent, OH

Join us as the Supervisor of OhioRISE Case Management/Care Coordination where you will lead a Care Coordination team that drives positive change in the lives of youth and families within our community. This is a full-time position based out of Kent, Ohio that will be serving Portage County. Great opportunity for those with a background in social work, social services, or other mental health related fields.

Your Impact as a Supervisor of OhioRISE Case Management/Care Coordination:

  • Provide strategic oversight for structured service planning and care coordination, centered on high-fidelity wraparound principles.
  • Develop and supervise a team of eight Care Coordinators.
  • Actively engage and develop relationships with child-serving systems and behavioral health providers.
  • Initiate quality improvement initiatives that align with OhioRISE program requirements.
  • Support the development of the local system of care serving youth and families.

Qualifications of a Successful Supervisor of OhioRISE Case Management/Care Coordination:

  • Licensed Social Worker (LSW), Licensed Professional Counselor (LPC), or higher.
  • Demonstrated supervisory or managerial experience in community-based services.
  • Bring a minimum of 3 years of experience in children's mental health or related public sectors, providing community-based services to children, youth, families, or caregivers.
  • Display experience in managing complex cases involving multi-system youth.
  • Monitor timely and accurate documentation and coach staff to performance.
  • Maintain an outstanding driving record.
  • Ability to obtain NPI and Ohio Medicaid numbers (assistance available).
