School Navigator

Coleman Health Services, a nationally recognized not-for-profit mental health and substance use services provider is seeking a full time school navigator to serve as a collaborative partner between students, families, schools, and behavioral health providers to support student wellness, educational success, and access to care. The School Navigator will focus on early identification, prevention, intervention, coordination of services, and linkage to community resources for youth experiencing behavioral health, substance use, social-emotional, or related challenges. The School Navigator will work within school settings in Logan and Champaign counties while maintaining a neutral, objective, and student-centered approach.

Job Duties of School Navigator include, but are not limited, to:

  • Develop and maintain collaborative relationships with school administrators, counselors, teachers, support staff, students, and families.
  • Provide ongoing consultation and support to school staff regarding student programming, interventions, behavioral concerns, and social-emotional needs.
  • Participate in school staffing meetings, student assistance teams, IEP/504-related support meetings as appropriate, and other collaborative planning meetings.
  • Serve as a liaison between the school, family, and behavioral health providers to strengthen communication and continuity of support.
  • Provide case management and care coordination services to support student success across school, home, and community environments.
  • Conduct visits and provide support within natural environments, including school, home, and community settings as appropriate.

Qualifications of the School Navigator

  • Bachelor's degree in a social service-related field or three-years' experience
  • Willing to get NPI and Medicaid Billing number (we'll help!)
  • Maintain excellent driving record
