Transition Care Coordinator

Coleman Health Services, a nationally recognized not-for-profit provider of behavioral health and rehabilitation programs is seeking transitional care coordinator to assist with rapid engagement and crisis stabilization of Auglaize, Allen, and Hardin Counties youth and families seen by the Mobile Response Team. Emphasis placed on keeping them safe in their nature environment, increased coping skills, accessing/navigating community resources, and linkage to outpatient services.

Duties include:

  • Provide short-term, and follow up community-based case management to assigned families
  • Safety planning and increasing healthy coping skills
  • Develop treatment plans
  • Assist with accessing supports, resources, and outpatient services that will help stabilize the crisis and keep them safe in the community

Qualifications:

  • Bachelor's degree in a related field or three years related experience (includes internships and volunteer experience);
  • Have and maintain an excellent driving record;
  • Familiar with child development, family systems, trauma informed care, community resources, motivational interviewing, crisis intervention, and case management approaches.
  • Prefer someone with prior case management experience.
  • Need someone who is able to work flexible hours, and some on call hours.
