Coleman Health Services, a nationally recognized nonprofit provider of behavioral health and rehabilitation programs to foster recovery, build independence, and change destinies for individuals, families, and businesses in our community. We are seeking a Jail Navigator to join our team in Trumbull County, Ohio, where you will provide mental health services to individuals involved in the criminal justice system, coordinate discharge planning, coordinate behavioral health care in the jail and connect clients with essential community resources before and after release.

Job Duties:

• Conduct assessments, monitor symptoms, evaluate risks, and connect clients with mental health and substance use disorder services.
• Coordinate care and discharge planning up to 30 days before and 30 days after release from jail.
• Develop relationships with jail personnel, courts, probation offices, and community service providers to facilitate continuity of care.
• Assist clients with accessing housing, employment, transportation, behavioral health treatment, and other community resources.
• Provide advocacy, community outreach, and appropriate referrals following release.
• Maintain daily reporting and communicate with providers regarding clients' legal status, treatment needs, strengths, and barriers to release.

Minimum Qualifications:

• Relevant experience in case coordination or case management with adults receiving behavioral health services.
• Experience working with individuals with mental health and/or substance use disorder needs.
• Must have or be willing to obtain a National Provider Identifier (NPI) and Ohio Medicaid number.
• Must have and maintain an excellent driving record.

Preferred Qualifications:

• Experience providing case management services within the Trumbull County region.
• Familiarity with community-based resources and criminal justice systems.
