Residental Case Manager

Coleman Health Services, a nationally recognized nonprofit provider of behavioral health and rehabilitation programs to foster recovery, build independence, and change destinies for individuals, families, and businesses in our community. We are seeking a Residential Case Manager to join our team in Kent, Ohio, where you will provide intensive, case management services to individuals with severe and persistent mental health issues in our supportive living environments.

Job Duties:

  • Advocate for clients based on identified needs, including financial, housing, medical, legal, social, and environmental
  • Provide mental health intervention to help individuals eliminate barriers to maintaining education or employment
  • Coordinate treatment and intervention and educational efforts between providers and other organizations
  • Meet service and associated documents requirements contained in client Treatment Plans and Clinical Performance Review form
  • Complete progress notes for services provided within specified timelines
  • Other duties as assigned

Minimum Qualifications:

  • Three years' experience in mental health or related field or BA/BS in mental health or related field
  • Have/maintain excellent driving records
  • Have or willing to obtain NPI and Ohio Medicaid numbers
  • Must be able to pass BCI/FBI background checks and an MVR check

Preferred Qualifications:

  • Experience in case management preferred
