Coleman Health Services is seeking a compassionate, organized Youth Care Coordinator – OhioRISE to join our team.

In this role, you’ll help children and youth with complex needs connect to behavioral health services, community resources, schools, providers, and other supports. You’ll coordinate care, build relationships with families and service providers, and help ensure youth receive the right services at the right time.

This is a care coordination and case management position, not a therapy or direct patient care role. Approximately 75% of the caseload is completed in person, so you should enjoy getting out into the community and be comfortable traveling to meet families where they are.

What You’ll Do

• Coordinate services and support for youth and families
• Build relationships with families, providers, schools, and community agencies
• Connect families with behavioral health and community resources
• Advocate for youth and families
• Participate in care coordination, case management, and family-centered planning
• Follow up with providers and service team members
• Meet with families in their homes and community
• Complete electronic documentation and maintain accurate records

What You Bring

You must meet one of these education/experience combinations:

• High School Diploma/GED + 3 years working with children or youth
• Associate Degree + 2 years of experience
• Bachelor’s Degree + 2 years of experience
• Master’s Degree + 1 year of experience

Experience in behavioral health, social services, child welfare, education, foster care, case management, care coordination, family services, or community programs is valuable.

We’re looking for someone who is compassionate, resilient, dependable, organized, adaptable, tech-savvy, and comfortable working with youth and families facing complex circumstances. Experience with electronic health records or medical/behavioral health documentation is highly desired.
