CME Moderate Care Coordinator (Youth/Young Adults) - FT - Portage and Summit Counties, Ohio

If you want to make a difference, we want you on our team!  As a CME Moderate Care Coordinator, you will provide structured service planning and care coordination through high-fidelity wraparound as established by the national wraparound initiative.  You will provide care coordination services to no more than twenty-five OhioRise youth. We will be serving Portage and Summit Counties!

What are we looking for from you? 

·  Ability to conduct comprehensive assessments including the CANS assessment

·  Provide wrap-around services

·  Convene and facilitate child and family team meetings

·  Develop and implement child and family care plans

·  Perform referrals and linkages to appropriate services and supports

·  Timely completion and submission of documentation

·  Complete training programs provided by CABHCOE

What Qualifications will you need to be successful as a CME Moderate Care Coordinator?  

·  High school diploma or equivalent with 3 years' experience (or 2 years' experience with an associate's/bachelor's degree or 1 year of experience with a master's degree) in children's behavioral health, or a related public sector providing community-based services to children and youth and their families or caregivers

·  Ability to manage complex cases to work with children and families who are engaged in one or more child-serving systems

·  Able to effectively communicate with youth, caregivers, multi-disciplinary staff, and partner agencies

·  Have and maintain an excellent driving record

Preference given to the following candidates:

·  Associate's, Bachelor's or Master's degree.

·  LSW/LISW/LISWS, LPC/LPCC/LPCCS, MFT/IMFT/IMFTS

·  Has NPI and Ohio Medicaid numbers
