We are seeking an experienced Critical Time Intervention Case Manager to join our Crisis Services Team in Steubenville, Ohio.
Critical Time Intervention is a program that closely monitors individuals recently discharged from a psychiatric hospital or Crisis Stabilization Unit for up to 6 months following discharge. The CTI Case Manager assists at risk clients with accessing community-based resources, housing, employment, making appropriate referrals and assisting with maintaining scheduled appointments.
Must have BA/BS in mental health or related field and be passionate about working with our clients; have or willing to obtain a NPI and an Ohio Medicaid number; and have and maintain an excellent driving record. LSW/LPC preferred.
