Case Manager (I Promise Health) - FT - Summit County, OH

Coleman Health Services, a nationally recognized not-for-profit provider of Behavioral Health and rehabilitation programs is seeking a bachelors level QMHS, Case Manager to provide client and community support. Highly skilled Case Manager working alongside collaborates with the I Promise Health-Quarters, which include LeBron James Family Foundation, Peg's Foundation, Akron Public School and AxessPointe.

Duties include:

  • Provide case management and wraparound services to youth and their families
  • Provide community-based care in a variety of settings connected to all Summit County APS students and LeBron James Family Foundation, supported facilities
  • Have strong clinical documentation skills and be able navigate documentation within an electronic health record
  • Work together with a team of counselors/social workers, case managers, peer support specialist and the Director to provide prevention, sustainability, and reduce trauma in youth and their family members
  • Be a member of the newest and most valued service line for youth as we increase awareness and bring hope to Summit County Youth and their families.
  • Be a member of a program striving for National Recognition, with heavy emphasis on cultural humility and trauma-based care.

The Summit County Coleman Team is seeking a new Case Manager for The I PROMISE Health-Quarters that will be a replicable model of care specifically designed to provide the best outcomes for students and their families in Akron. Qualifications:

  • Bachelor's degree in a social service-related field OR 3 years' experience in a similar position
  • Willing to get NPI and Medicaid Billing number (we'll help!)
  • Maintain excellent driving record
