Housing Case Manager - FT -Portage County

Coleman Health Services is seeking an individual to provide case management services to individuals with severe and persistent mental health issues,. The Housing Case Manager will assist with accessing community based resources, housing, employment, making appropriate referrals and linking the individual with ongoing mental health services. This position will be focused on addressing ongoing issues related to obtaining and maintaining housing and work with homeless households, as well as those living in our apartments and supportive living sites.

Qualifications of a Housing Case Manager:

  • Three years' experience in mental health or related field or BA/BS in mental health or related field
  • Have/maintain excellent driving record
  • Have or willing to obtain NPI and Ohio Medicaid numbers
  • Experience in case management preferred
