Based on Coleman's 40-year record of success, we were recently awarded a nearly $4 million SAMHSA grant to implement Certified Community Behavioral Health Clinics (CCBHCs) in our service areas of greatest need. Coleman is seeking individuals to provide intensive, case management services to individuals with severe and persistent mental health issues in Portage County, Ohio. We have two different kinds of Case Managers we are seeking:
•Case Manager will assist with accessing community based resources, housing, employment, making appropriate referrals and linking the individual with ongoing mental health services.
•Case Manager Assertive Community Treatment (ACT) - ACT is an evidenced-based, shared-caseload, multi-disciplinary practice of delivering comprehensive community based behavioral health services to adults with certain serious and persistent mental illnesses.
Qualified candidates will have a BA/BS in mental health or related field or three years' experience in mental health or related field, have a NPI number, and an Ohio Medicaid number. Must have and maintain excellent driving record. Experience in case management preferred.Benefits options include health, life, pension, paid time off, and more.
