Coleman Health Services is seeking a full-time Guardian Representative/Case Manager to coordinate provision of legal guardianship services to adults in Ohio. Individual will be responsible for coordinating and implementing efforts to assist clients in maintaining optimum health care, safe living environments, and continued quality of life.  Individual will work closely with other community professionals to coordinate needed services to support the guardianship client and frequently be required to present in a court or legal setting. 

Must have a Bachelor's degree related to social service or medical field.  Knowledge of community resources, previous work with probate court and guardianship services preferred. Knowledge of older adult resources and community services a plus.
