Housing Specialist

Coleman Health Services is seeking an individual to provide case management and housing services to individuals with severe and persistent mental health issues, living within Trumbull County, Ohio. The Housing Specialist will be responsible to manage households related to a Continuum of Care grant. The Housing Specialist will be determining eligibility and providing continuity of care to the families on the grant. The Housing Specialist will assist with community based care coordination.

Qualified candidates:

  • Three years' experience in mental health or related field or BA/BS in mental health or related field
  • Have/maintain excellent driving records
  • Have or willing to obtain NPI and Ohio Medicaid numbers
  • Experience in case management preferred
