Crisis Outreach Specialist - FT - Portage County, OH

Coleman Health Services is seeking diligent and responsible individuals to join our customer-centered emergency crisis team of professionals to work with individuals experiencing immediate mental health crises in Portage County, Ohio. This position assists in client engagement and obtaining needed treatment, including emergency intakes, assessments for psychiatric hospitalization and linkage to community services and resources. This position will also work closely with law enforcement and assist on engaging at risk clients released from jail and prison.

Duties include:

  • Review and complete mental health outreaches requested by police
  • Collect and review mental health outreach data and grant outcomes
  • Assist in engaging clients with mental health treatment who are released from jail or prison
  • Determine needed clinical services, level of urgency, and appropriate providers; consult with personnel and assist with scheduling as needed
  • Provide supportive and/or basic crisis intervention in person and via phone
  • Receive and coordinate external referrals to Access (i.e. from Law Enforcement)

Qualifications:

  • BA in related field, LSW preferred
  • Clean driving record
  • Have or willing to obtain NPI and Ohio Medicaid numbers
