Case Manager (Youth/Young Adult) - FT - Portage County, Ohio

Coleman Health Services, a nationally recognized not-for-profit provider of behavioral health and rehabilitation programs, is seeking a social work or counseling individual to provide intensive, case management services to children, adolescent, and young adults (up to age 26) with mental health and substance use issues in Portage County Ohio. Make a difference in the lives of children and families at risk by assisting them with accessing community-based resources such as housing, medical and financial benefits, employment, coordinating care with local schools and other outpatient providers, making appropriate referrals, and linking the individual with ongoing mental health services.

Duties include: 

-  Assist with accessing community-based resources, housing, and employment

-  Making appropriate referrals

-  Linking the individual with ongoing mental health services

Qualifications: 

-  BA/BS in mental health/related field or three years' related experience

-  Have/obtain a NPI number and an Ohio Medicaid number

-  Have/maintain an excellent driving record

Preference given to the following candidates:

-  BA/BS in mental health/related field

-  Experience in case management

-  Experience working with children and young adults in a community mental health setting
