Re-Entry Engagement Specialist

Seeking a Community Transition/Re-entry Specialist to work with individuals who are transitioning from being incarcerated back into the community with a focus on establishing a lifestyle of recovery. The successful candidate will establish discharge planning, assess need and risk, monitor symptoms and provide therapeutic behavior support services according to Medicaid guidelines, and link to all needed resources and services that are identified.

Must have a BA/BS in a related field plus one year experience in mental health or related field. Must have Ohio driver's license, clean driving record, and willingness to transport clients. Must have or be willing to obtain the following: an NPI number and an Ohio Medicaid number. Excellent communication and team work are key characteristics.
