Join us as the Director of OhioRISE Case Management/Care Coordination (Youth/Young Adults)based out of our Akron office leading a specialized managed care program that focuses on children and youth with complex mental health and multisystem needs across Summit county, Ohio.

As the driving force behind our mission, you'll provide strategic oversight for structured service planning and care coordination, centered on high-fidelity wraparound principles. Leading a diverse team of Intensive and Moderate Care Coordinators, you'll ensure the delivery of exceptional mental health services. Additionally, you'll actively engage and support our community partners.

What We Seek in a Director of OhioRISE Case Management/Care Coordination (Youth/Young Adults):

  • Provide orientation, guidance, and supervision for a team of eight care coordinators.
  • Monitor and ensure ongoing training, coaching, and support for consistent care coordination for multi-system youth and their families.
  • Embrace high-fidelity wraparound principles as a guiding philosophy.
  • Communicate effectively with OhioRISE youth, families, community providers, system partners, and stakeholders.
  • Engage in quality improvement initiatives aligned with OhioRISE program requirements.
  • Foster consultation, liaison, and program education with community agencies and organizations.
  • Engage seamlessly with a diverse workforce and various levels of Coleman staff.
  • Maintain timely and accurate documentation.

Qualifications of a Successful Director of OhioRISE Case Management/Care Coordination (Youth/Young Adults):

  • Licensed Social Worker (LSW), Licensed Professional Counselor (LPC), or higher.
  • Demonstrated supervisory or managerial experience in community-based services.
  • Bring a minimum of 3 years of experience in children's behavioral health or related public sectors, providing community-based services to children, youth, families, or caregivers.
  • Display experience in managing complex cases involving multi-system youth.
  • Maintain an outstanding driving record.
  • Possess or be willing to obtain NPI and Medicaid numbers (assistance available).
