Case Manager/Eldercare Navigator (Social Services) - FR- Stark County

The Case Manager Eldercare Navigator will assist with accessing community based resources, housing, employment, making appropriate referrals and linking the individual with ongoing mental health services and other services as needed.  Must have BA/BS in mental health or related field or five years' experience in mental health or related field.  Must have and maintain excellent driving record.
