Must have an LISW or LISW-S, LPCC or LPCC-S in mental health field plus indirect service in an integrated primary behavioral health care setting and experience with SMD population. Must be eligible to obtain an NPI number, enrolled as a provider to bill Ohio Medicaid. Must have CAQH account. Must also be able to communicate effectively with consumers, families, multi-disciplinary staff, key stakeholders and partner agencies in addition to having at least 3 years' experience and training in serving the SPMI and SMD, SUD and co-occurring disorders populations. Experience in budgeting and/or program evaluation are also preferred. Salary based on experience and credentials.
